How to register

The practices within Port Glasgow Health Centre operate a rota  for registering new patients with their practice. Patients living in or moving to the practice area are welcome to join. The practice has a contract with the Health Board to look after patients in the postal code areas PA13, PA14 and PA15 2. Any current patients living outwith these areas will remain registered here. Any current patients that move outwith these postal code areas will be required to change to a doctor within their new post code area.

You can now apply to register at our practice online by clicking here and downloading an application form. Once you have completed the form you can send it as an attachment to; ggc.gp86321clinical@nhs.scot or print it off and post it to the practice. You will receive confirmation that your form has been received and whether your application to register will be accepted or not. The next step is a registration appointment with the practice nurse, due to the current pandemic the consultation may be carried out over the phone. This will be discussed with you once your form has been submitted and accepted.
